Care Coordination: Increasing Access to Improve Health Outcomes

Planned Parenthood (PPM) finds that many patients, who initially present for reproductive health needs and consultations, do not have primary care physicians; thus access to PPM services is many times the entry point that identifies other vital health and social service needs. Currently, many patients are referred to other needed services, but there is minimal follow-up to ascertain whether or not these services were received and completed.

The Baltimore Women's Giving Circle grant will help support the salary of a new position, Care Coordinator, filled by a licensed social worker to help ensure that all needed services are completed.
