SMOC clients are older homeowners living alone, grandparents raising children, residents of senior housing apartments, and those at risk of becoming homeless. For older adults, meeting their financial, physical and emotional needs means aging on their terms by having money to afford basic needs, being physically healthy enough to stay in their homes, and having satisfying relationships and resilience when they face challenges. SMOC helps connect clients to public benefits; provides financial coaching, Medicare counseling and education, and clinical care coordination. In addition, SMOC teach skills to help process emotions, to communicate with loved ones, and to build tools for resiliency.

BWGC funds supports ongoing case management activities.
